To successfully complete the claim form:
Under "Organization/League name", write: "USA Football- [your organization name]"
Under "Policy Number" Input the policy number found in your Participant Accident Medical Coverage in your Certificate of Insurance (COI). See screenshot below for where to find this number.

The claimant will hear from Health Special Risk (Your claims provider) within 15 business days from the date of claims submission.
